That remote looks like it’s for an adjustable bed base, often used with brands like Ergomotion, Sleepmotion, or Tempur adjustable bases. The layout — with separate controls for Head, Pillow, Lumbar, Foot, and Massage — is quite common among models sold under different names but made by the same manufacturers.
If you check the underside or back of your bed base, there’s usually a model label or serial sticker. That’ll confirm the exact brand and model it belongs to.
From the button layout, it seems closest to the Ergomotion 4500/5100 series or Dream Motion-style bases.
That design matches pretty closely the remote used by InnoMax for their “Prodigy” adjustable power‑base.
